With due respect I am to inform you that I am ready to attend my duty as desired by our Department during the total Lock down orders for containment of spread of COVID-19 epidemic throughout the Nation.

But in total absence of Public transport including Rail during this lock down period it is quite impossible to reach my respective office.

With due respect I am to inform you that I am unable to attend my duty during the total Lock down orders for containment of spread of COVID-19 epidemic throughout the Nation as I am feeling physically ill ( or any other valid reason eg taking care of my child or taking care of my old parents etc.) and I desired to kept myself in Quarantine at home.

Therefore, it is requested to kindly allow me to keep quarantine myself during this total Lockdown period.
