Removal of unwanted IPs from the list of AV rolled out systems

Many Old PCs IP addresses are still available in AV Monitoring Server which are not in use or condemned. This will affect the overall performance of AV updation status in the Division/Region/Circle. Hence, the above proforma may be used to remove IP address from the list of AV Rolled out systems.
